Yes, you will have to complete 24 hours of continuing education during the two-year period before renewal. If you complete more than 24 hours of continuing education, you may not carry over the excess hours into your next renewal period.

You cannot work as a massage therapist until you are issued a license by the Board.
You should make sure your application is filled out correctly and completely and ensure any required documents are sent as quickly as possible.
If you attended a school in Arizona that is accredited by an agency recognized by the Secretary of the U. S. Department of Education, you are exempt from taking an examination. Otherwise, you must take the examination found at www.ncbtmb.com or www.fsmtb.org
You may obtain a license if you meet all of the requirements for licensure, including attendance at a board recognized school. A massage therapy school located in another state must apply for approval to the Board of Massage Therapy. The school must show that it is accredited by an agency recognized by the secretary of the U. S. Department of Education or is approved by an agency similar to Arizona’s Private Postsecondary Education.
An applicant is eligible for reciprocal licensure if either of the following applies:
- The applicant has been licensed continuously in another state for the last five years preceding the filing of the application with the Board and the other state has standards for massage therapists that are substantially equivalent to those in this state. OR
- The applicant holds a current certification from the national certification board for therapeutic massage and bodywork or another agency that meets the standards of the national commission on competency assurance and received education and training substantially equivalent to that required by this chapter.
You must submit a name or address change in writing within 10 days after the change. You may email the change to [email protected] or send a form, which is available on the Board’s home page under “Forms.” If you want to make a name change, you must submit proof of the change, such as a copy of a marriage certificate, divorce papers, or official court documents. There is no charge for a name or address change.
Yes, there is a $25 fee, payable by money order or cashier’s check for a replacement license.
No there is no list. The requirements for approval of CE can be found in the Board’s rule A.A.C. R4-15-302, which are also located on the Board’s website under the Statutes and Rules tab. Please review the rule before you take a course to ensure that the course is from an approved provider.

You can request that the board office mail you one if you download the application or you can come pick up one from the office. DO NOT BEND THE CARDS.
You can have your fingerprints rolled at any law enforcement agency or any other agency that performs fingerprint services. Please contact them for times and fees. The fingerprinting does not need to be done in Arizona, but you must use a card that has “APPLICANT” on the left hand corner.
Review the statutes for the definition of a massage therapist. If you meet the definition you must be licensed.
