Print a hard copy of the State licensing forms, complete the sections by pen, & submit with fee payment by U.S. mail.

Processing begins when forms & fee payment are received.

After the current legislative session, all new laws impacting state regulation will be reviewed by the Arizona Attorney General's Office before implementation.

Renewing Your State License?
State law permits License Renewal
up to 60 days - before - expiration.
*Arizona State law requires completion of 24 Continuing Education hours before submitting a license renewal application.
The current COVID waiver permits all these hours to be taken virtually, until the last day of 2022.
Beginning January 1, 2023, half of the 24 CE hours may still be taken online.
Per state law, the other half of required hours must be taken in-person.
NEED HOURS?
LMT's attending any State Board meeting (virtually), can receive 3 free CE hours once a year.
Check Board website for upcoming meeting dates. (Special/Emergency meetings not included for CE credit). Provide AZ license number via email, at conclusion, for CE credit hours.
