If I already have my license, how do I change my name or address?
You must submit a name or address change in writing within 10 days after the change. You may email the change to [email protected] or send a form, which is available on the Board’s home page under “Forms.” If you want to make a name change, you must submit proof of the change, such as a copy of a marriage certificate, divorce papers, or official court documents. There is no charge for a name or address change.
