NEW LICENSING REQUIREMENT BY ARIZONA LEGISLATURE 

All Arizona therapists are now required to have current DPS Level-One fingerprint clearance card for legal practice.

License renewals cannot be processed without DPS-issued card.

FOR REQUIRED NATIONAL AND STATE CRIMINAL BACKGROUND CHECKS,  YOU PROVIDE TWO INKED FEDERAL FINGERPRINT CARDS;  ONE TO AZ DPS, ONE TO BOARD

At DPS website, complete their application form, providing them a second inked-original FD-258 fingerprint card.  Fee payment is directly to DPS.

(EXISTING LEVEL-ONE HOLDERS, NEED ONLY REGISTER ACTIVE MASSAGE LICENSE WITH DPS)

When DPS issues, or updates, your existing Level-One clearance card, scan and email a copy to the Board.

Do not send your original clearance card to the Board
