Arizona State License Renewal?

      NEW LICENSING REQUIREMENT BY ARIZONA LEGISLATURE 

All Arizona therapists are now required to have a current DPS Level-One fingerprint clearance card for legal practice.

License renewals cannot be processed without DPS-issued card.

At DPS website, complete their application form, providing them a second inked-original FD-258 fingerprint card.  Fee payment is directly to DPS.

(EXISTING LEVEL-ONE HOLDERS, NEED ONLY REGISTER ACTIVE MASSAGE LICENSE WITH DPS)

 

When DPS issues, or updates, your existing Level-One clearance card, scan and email a copy to the Board.

Do not send your original clearance card to the Board.  

   

 

Avoid Lapse in License & Missed Work  --  Renew Early

Apply when within 60 days from expiration. 

#1 Complete the online license-renewal application form,

     attaching the required proof of citizenship/visa, CE certificates, submit online license fee payment.

#2 Obtain and attach current/or new copy of your DPS Level-One fingerprint clearance card.

#3 Submit online at  https://massagetherapy.az.gov/ for processing.

Attach copies of CE certificates, Total of 12 CE hours may be online; but the remaining 12 hours are State-required to be in the classroom, physically, with the instructor.  (Live Webinars not a substitute). Instructors provide attendance verification. 

Since 2003, Arizona law requires completing 24 Continuing Education hours before submitting a license renewal application.
