Initial Arizona Massage License

       NEW LICENSING REQUIREMENT BY ARIZONA LEGISLATURE 

All Arizona therapists are now required to have a current DPS Level-One fingerprint clearance card for legal practice.

License initial applications cannot be processed without DPS-issued card.

 

FOR REQUIRED NATIONAL AND STATE CRIMINAL BACKGROUND CHECKS,  YOU PROVIDE TWO INKED FEDERAL FINGERPRINT CARDS;  ONE TO AZ DPS, ONE TO BOARD

At DPS website, complete their application form, providing them a second inked-original FD-258 fingerprint card.  Fee payment is directly to DPS.

(EXISTING LEVEL-ONE HOLDERS, NEED ONLY REGISTER ACTIVE MASSAGE LICENSE WITH DPS)

 

When DPS issues, or updates, your existing Level-One clearance card, scan and email a copy to the Board.

Do not send your original clearance card to the Board.

For expedited licensing, submit your initial application, fee payment and inked-Federal print card to State Board

& apply for State license up to 120 days before graduation.  (Mblex testing mandatory for any Arizona program not COMTA nationally-accredited).

 

-The State of Arizona requires a completed initial license application form, submitted online at Board website. Applicant provides  a current a color photograph, (passport-style), the authorization number issued after online credit card fee-payment, and a hard-copy and inked FD-258 Federal fingerprint card.    

-The State Board is now required to obtain a scanned copy of an Arizona Department of Public Safety Level-One (Non-IVP) clearance card before license application processing.

-Student transcripts can only be accepted directly from the school program.

-National Mblex test results come directly from FSMTB. 

-All out-of-state licenses, prior and current, require that state's written license verification.

 

NOTE:  Applicants for an Arizona massage license, from California's voluntary program, do not qualify selecting "Reciprocity" on their application.

Current FBI background check averages 12 weeks.

 

 By law, the Arizona Massage Board must wait for Federal national background clearance & DPS Level-One card copy before licensing.

To avoid returns, all inked FD-258 Federal fingerprint cards, must have completed fieldsincluding applicant's valid Social Security number, and legal signature for Federal processing.
